Editor's note: As a point of information, it is not uncommon for us to receive numerous revisions of these entry forms, and usually the revision goes unannounced - that is, we just put the new form up. Folks who are planning to enter youth tournaments are advised to recheck the entry forms periodically to see if anything has changed. We don't mind posting revisions, but we cannot be responsible for any incorrect or old forms which may be circulating - we have no way of getting them all back.
