Editor's note: As a point of information, it is not uncommon for us to receive numerous revisions of these entry forms, and usually the revision goes unannounced - that is, we just put the new form up. Folks who are planning to enter youth tournaments are advised to recheck the entry forms periodically to see if anything has changed.
